Credit Card Authorization Form | 2024-2025 Dance Season

Tuition will be calculated and available to view online in the "Parent Portal" on or before the first of the month. You are able to check your balance at any time by logging into your account in the "Parent Portal" on our website.

I understand that monthly tuition is calculated by total length of classes my dancer participates in each week and will be billed in (8) eight equal monthly payments between September and April. I understand that if I register after September 30 2024, I will be required to pay full tuition during the month of May, 2025. I understand that additional fees such as costumes, recital package fee and rehearsal fees will be added and withdrawn fom my account, when applicable. By completing and signing this authorization form; I agree to authorize Centerstage Dance Academy to automatically charge the credit card on file, on the 5th of every month, from October, 2024 through April, 2025 and/or May 2025 (if applicable). I understand that the September tuition charge will be withdrawn on Monday, September 16 due to the start date of the season.

I understand that if my payment is declined for any reason, a $10 declined credit card fee will also be applied and charged to my account.I understand that if my credit card is declined for any reason, I must give an updated card within 48-hours or a $25 late fee will be charged to my account and my dancer may be unable to continue participating in dance classes.

I understand that I am able to check my balance at any time by logging into my account using the "Parent Portal" which is available on our website. I understand that Centerstage Dance Academy reserves the right to discontinue the account of any customer with an outstanding payment balance of 60 days or more. Accounts and unpaid balances that are more than 60 days past due will be submitted to a collections agency. I understand that Centerstage Dance Academy will not be responsible for any attorney or collection fees arising from our efforts to collect unpaid balances. I understand and agree that if any changes occur to the credit card listed below, I will update this form in a timely manor so that Centerstage Dance Academy has a valid credit card on file for my account. I understand that I must submit, in writing, a request to opt-out of auto-pay BEFORE the 5th of the month to stop payments from being automatically withdrawn.

If you do not want to list your credit card information here - please add your credit card information directly into your Parent Portal. You must still submit this form to become enrolled in autopay, but instead of writing your entire credit card number, you can confirm/write the last 4-digits of the card on file in the “Credit Card Number” section of the form.
